Fees and Insurance
To help control costs, we ask that you pay any deductible and co-payments at the time of your visit. This helps us avoid costly, unnecessary billing procedures.
- Insurance forms: We will be happy to file your primary insurance form. Any secondary insurance will be your responsibility.
- Medicare: We accept assignment on all Medicare claims. Please note: As required by law, you will be required to pay your deductible and 20% co-payment.
- Blue Shield: We participate in all BC/BS plans.
- United Healthcare, Cigna and most other health plans accepted.
